Write a brief summary of the issue in precise language to clearly state the issue.



Expand the content of the Issue Summary into details that clearly and precisely state your observations. Your description should include a list of exact steps leading to the occurrence of the issue. If you are not able to reproduce the issue after the original occurrence try to state the steps as you remember them best. Attach to your email any screenshots, log file, templates, or any other supporting information that helps in diagnosing the issue.



Rank the priority of the issue on a scale of 1-5 with 1 being the highest priority and 5 the lowest priority. Highest priority should be assigned to issues that are severe and/or impact many users. Lowest priority is reserved for low importance issues including minor cosmetic issues. Between highest and lowest priority are varying levels of severity.
